HOW
DO I GET STARTED WITH THE CLUB?

Go
through the online
catalog or send your name and address to carla@usborne-texas.com
for a complete catalog to be mailed to you and make a list of all the
books that you want. Print
this form, fill it out by month, writing down the titles you want
under the month you want to receive them. Please complete the shipping
information completely. Remember and plan a month ahead for
birthdays, holidays and special occasions!

Mail
or email me your completed form and your shipments will start within
the month!

Your
credit card will be charged monthly or you may pay by check. Payment
must be received before order can be placed on the first of each
month. The orders should arrive around the 15th of each
month. Shipping
charges are $3.00 per month per shipment. If you have more than
one monthly shipment, i.e. to 2 different grandkids in two different
locations, then $3.00 shipping is charged on each one.
